Pet License Registration

THIS PET LICENSE REGISTRATION FORM IS FOR CALENDAR YEAR 2023. THANK YOU!

Pet License Registration is effective for the current calendar year. Please come back and fill out this form each January to receive a new registration tag for that calendar year. A copy of the updated rabies vaccination certificate must be attached to the registration for each pet.

After you have completed filling out this form you will be directed to the Easy Pay payment screen to complete your payment with credit card. The registration fee is $15.00 for altered animals and $25.00 for unaltered animals (+ a processing fee).

If you are registering more than one pet and at least one pet is altered and one is not, please contact City Hall to process your fee payment with assistance: 515-993-4525.
